Understand

Falls from height are among the most common causes of serious workplace injury. Even a short fall from a stepladder can break bones, cause head injuries or worse. Monthly inspections catch deterioration before it causes a fall.

Worn feet and faulty locking mechanisms are the most critical failure points. Rubber feet that have worn smooth dramatically increase slip risk, and spreader bars or locks that do not engage properly can cause the ladder to collapse suddenly during use. You must physically set up the ladder and test it, not just look at it. Verify all locking mechanisms engage under tension, check each rung for looseness, and try to move each foot by hand to confirm it will not pull off unexpectedly.

A failed ladder must be physically removed from where it might be used, clearly marked with a condemned tag, and stored securely. Do not leave failed equipment where someone might use it anyway. Poor storage causes many of the defects found in inspections, so ladders should be stored horizontally on brackets or hooks, protected from weather and not in walkways where they can be hit by trolleys.
